I don’t want to leave you hanging waiting on an answer
I am working on a couple of solutions, probably a bat file and a reg file.
How ever it is time to close the shop and head for home, I’ll be back in a couple of hours.

We need to find the hidden file that is making the random.dll file
First download
Start Dllcompare and click on "Locate.com" and allow the scan to complete.
After the scan has finished click on "Compare" to scan for hidden files this step will take a few minutes to run.
Click on ‘Make a Log of what was Found’, when prompted to "View Log File" click on "Yes"
Leave the notepad window open you need to copy the text later.

[b]Now start HijackThis and click the ‘Open the misc tools section’
check both the boxes next to the Generate a startup list log button
now click ‘Generate a startup list log’

Now copy and paste the text from both the Dllcompare results and the startup log from Hijackthis into this thread.

Please copy this to Notepad and save it as a text file you will need it later to copy and paste from.
Make sure to work through the fixes in the exact order it is mentioned below. If there's anything that you don't understand, ask your question(s) before proceeding with the fixes.
You should not have any open windows when you are following the procedures below.

Boot into safe mode:
Restart Windows, after you see the BIOS screen and before Windows starts to load.
Start tapping the F8 key. The Windows Advanced Options Menu appears.
Use the Arrow key to ensure that the Safe Mode option is selected.

Once it has booted into safe mode click start > run type the bellow into the text box and click OK:
regsvr32.exe C:\WINDOWS\system32\dxnlobby.dll /u

Open the text file you saved from above

Close all other open Windows and have HiJackThis Fix:
First copy the random named file path (C:\WINDOWS\system32\some random name.dll)
and paste it into the text file you saved above

O20 - Winlogon Notify: RunOnceEx - C:\WINDOWS\system32\some random name.dll
O23 - Service: Command Service (cmdService) - Unknown owner - C:\WINDOWS\QWRtaW5pc3RyYXRvcgAA\command.exe (file missing)

Now run KillBox and paste the below into the text box:
C:\WINDOWS\system32\dxnlobby.dll
Now put a check next to 'Unregister dll before deleting' and next to ‘Standard file kill ‘
and next to ‘End explorer while killing file’ click on the red X, click yes to delete file
Do the same for each of the following files

C:\WINDOWS\system32\some random name.dll
C:\WINDOWS\SYSTEM32\cmusapi.dll
C:\WINDOWS\SYSTEM32\cxyptui.dll
C:\WINDOWS\SYSTEM32\hrju05~1.dll
C:\WINDOWS\SYSTEM32\lvjo09~1.dll
C:\WINDOWS\SYSTEM32\lvl209~1.dll
C:\WINDOWS\SYSTEM32\mktext40.dll
C:\WINDOWS\system32\lvl0093me.dll

If you can not delete any of the above items:
run KillBox and paste the file path into the text box:
Now put a check next to 'Unregister dll before deleting' and next to ‘Replace file on reboot” and “Use dummy file”
click on the red X, click yes to delete file and click no to reboot now
do the same for each file you could not delete

Open up C:\windows\wininit.ini with notepad and remove the file path to be renamed,
save the wininit.ini file and close notepad

Click start > run type the bellow into the text box and click OK:
%temp%
Delete everything inside this directory that comes up

Goto Start > Control panel > Internet options > in the Temp internet files section
click the Delete file button, put a check next to Delete all off line content click OK
then empty your Recycle Bin, reboot and post a new HiJackThis log.

I followed your instructions but there were som steps that didnt work out:
1) Once it has booted into safe mode click start > run type the bellow into the text box and click OK: regsvr32.exe C:\WINDOWS\system32\dxnlobby.dll /u - this module did not exist

2)Open up C:\windows\wininit.ini with notepad and remove the file path to be renamed,
save the wininit.ini file and close notepad
- this file did not exist

3)Click start > run type the bellow into the text box and click OK:
%temp%
Delete everything inside this directory that comes up
- Cookies, History, and Temp. Int Files Folders could not be deleted because "index.dat" was being used by another program

Here is a new hjt log.

Please copy this to Notepad and save it as a text file you will need it later to copy and paste from.

Physically disconnect from the internet (unplug the wire)

Boot into safe mode:

Go to My Computer->Tools/View->Folder Options->View tab and make sure that 'Show hidden files and folders' is enabled. Also make sure that Display the contents of System Folders' is checked.

Click start > run type the bellow into the text box and click OK:
services.msc
In the right side, scroll down to Telephony right click on it and select ‘Stop’

Open up C:\windows\wininit.ini with notepad and remove the file path to be renamed,
(C:\WINDOWS\system32\some random name.dll ) save the wininit.ini file and close notepad

Close all other open Windows and have HiJackThis Fix:
First copy the random named file path (C:\WINDOWS\system32\some random name.dll)
and paste it into the text file you saved from above

O20 - Winlogon Notify: RunOnceEx - C:\WINDOWS\system32\some random name.dll
O23 - Service: Command Service (cmdService) - Unknown owner - C:\WINDOWS\QWRtaW5pc3RyYXRvcgAA\command.exe (file missing)

Now run KillBox and paste the below into the text box:
C:\WINDOWS\system32\ some random name.dll
Now put a check next to 'Unregister dll before deleting' and next to ‘Standard file kill ‘
and next to ‘End explorer while killing file’ click on the red X, click yes to delete file.

Click start > run type the bellow into the text box and click OK:

%temp%
Select the index.dat file (highlight) now go to ‘Edit’ > ‘Inverse selection’ and hit the delete key.

Go to Start > Control panel > Internet options > in the Temp internet files section
click the Delete file button, put a check next to Delete all off line content click OK
then empty your Recycle Bin, reboot and post a new HiJackThis log.

I think you have the newest version of VX2. Download L2mfix from one of these two locations:

http://www.atribune.org/downloads/l2mfix.exe
http://www.downloads.subratam.org/l2mfix.exe

Save the file to your desktop and double click l2mfix.exe. Click the Install button to extract the files and follow the prompts, then open the newly added l2mfix folder on your desktop. Double click l2mfix.bat and select option #1 for Run Find Log by typing 1 and then pressing enter. This will scan your computer and it may appear nothing is happening, then, after a minute or 2, notepad will open with a log. Copy the contents of that log and paste it into this thread.

IMPORTANT: Do NOT run option #2 OR any other files in the l2mfix folder until you are asked to do so!

Close any programs you have open since this step requires a reboot.

From the l2mfix folder on your desktop, double click l2mfix.bat and select option #2 for Run Fix by typing 2 and then pressing enter,
then press any key to reboot your computer. After a reboot, your desktop and icons will appear, then disappear (this is normal).
L2mfix will continue to scan your computer and when it's finished, notepad will open with a log.
Copy the contents of that log and paste it back into this thread, along with a new hijackthis log.

IMPORTANT: Do NOT run any other files in the l2mfix folder unless you are asked to do so!

I think that it is being rewriten by a temp file so some new instruction at the end.

Please copy this to Notepad and print it. Make sure to work through the fixes in the exact order it is mentioned below. If there's anything that you don't understand, ask your question(s) before proceeding with the fixes.
You should not have any open windows when you are following the procedures below.

Download and extract but do not run

Boot into safe mode:

Go to My Computer->Tools/View->Folder Options->View tab and make sure that 'Show hidden files and folders' is enabled. Also make sure that Display the contents of System Folders' is checked.

Turn off system restore by right clicking on My Computer and go to Properties->System Restore and check the box for Turn off System Restore. Click Apply and then OK.
Close all other open Windows and have HiJackThis Fix:

O20 - Winlogon Notify: RunOnceEx - C:\WINDOWS\system32\some random name.dll
O23 - Service: Command Service (cmdService) - Unknown owner - C:\WINDOWS\QWRtaW5pc3RyYXRvcgAA\command.exe (file missing)

Now run KillBox and paste the random file name into the text box:
C:\WINDOWS\system32\ some random name.dll
Now put a check next to 'Unregister dll before deleting' and next to ‘Delete on reboot ‘
click on the red X, click yes to delete file click no to reboot now

Now run GetPath.exe, Click Get path, Click Copy Path, click exit
Now Click Start > Run, Right click on the text box and select "Paste" click OK
Delete every file and folder in the window BUT the index.dat and desktop.ini files.

Click Start > Run and copy the below into the text box:

%temp%
Delete every file and folder in the window except index.dat.

Empty your Recycle Bin, (cross your fingers) reboot and post a new HiJackThis log.

After looking through the posts here I think you ran dllcompair instead of the l2mfix your log should have been much longer.
