Upgrading OptiPlex GX755 Mid-Tower

I recently got a OptiPlex GX755 Mid-Tower off of Craigslist for 100$, and I would like to upgrade it to perform better on games.

Here are the current specs -
CPU - Intel Core 2 Duo 2.66 GHz
RAM - DDR2 667 MHz 2GB x 2 --I really want to up this to either 1066 MHz or 866 MHz with 4GB or 8GB, badly.
Video - Radeon HD 2000 XT --Does fairly good on Minecraft with near max settings, but want to upgrade it to a better one.

I would like to upgrade the RAM and Video Card for better performance on my games. My question(s) are -
1) What's the best GPU for the buck?
2) What's the best RAM for the buck?

Some specifics on the RAM -
I would like to have 2GB x 4 sticks. I'm not sure, but I think this performs better if the RAM is spread out, but I am willing to go 4GB x 2 if it's cheaper and really doesn't effect performance.

Also, I don't want to go over 150$. So I am willing to go 1GB x 4 if necessary.
